To get started we need you to answer a couple questions before we meet for the first time….please write down the answers and email them back to me … be sure to also bring them with you when we meet…

 

 

 

Health:

  • What are your health and fitness goals… list them in priority from 1-10… (Jump higher, better endurance, regain optimal health, lose fat, build muscle, etc)?
  • What would be your realistic time frame of when you would like to achieve these results?
  • What does the activity level look like for you? 1-2 times per week or month etc?
  • What types of activity do you do?
  • Do you have any injuries that hinder you from physical activity?

 

What do you believe?

  • The harder and more you work the better results you achieve with you fitness goals, true or false?
  • Do you believe that making long lasting, sustainable changes in your health is hard or easy to do? What make you feel this way?
  • What amount of money would you say you spend on your health and wellness per month and on what? (Nutrition, Massage therapy, Chiropractor etc)
  • What hinders you “gets in the way” of achieving your life goals? (Physically, career, mentally, financial restrictions)
  • Have you ever had a personal trainer or lifestyle coach before? If so, describe your experience.

 

Stress level:

  • What do you do for a living? (Career)
  • Do you have children?
  • What does your financial support consist of? (Minimal or abundant? Job, family, marriage)

 

What do you consume?

 

·               Please write down what you have food consumption you have had in the past 3-4  

   days…please be as clear and open about this as possible, and include beverages as well?

·               How many times do you eat out (fast food) per week?

·               Do you drink alcohol? If so what is the consumption per week?
